Returning Home — Demonstration Project for Improved Discharge Planning and Continuing Care for Aboriginal Children with Complex Health Needs

Returning Home was a demonstration project supporting Aboriginal families, helping coordinate a child’s care needs regarding discharge planning and the continuum of care within the community. Returning Home was offered to Aboriginal families living in central and north Vancouver Island. As of May 2016, based on the success of the demonstration project and a clear need for health systems navigation, the First Nations Health Authority, in partnership with Is​land Health​, has established three Nurse Navigator positions based in First Nations communities on Vancouver Island. The Nurse Navigators will work to remove barriers and improve health outcomes by facilitating coordinated, culturally safe access to support, services and resources for First Nations.
